Please read these terms carefully before enrolling. By signing up for the SDC Membership Plan, you acknowledge that you have read, understood, and agree to all terms and conditions outlined below.
The SDC Membership Plan ("Plan") is an in-house dental discount membership offered directly by Smile Design Center ("SDC", "we", "us", "our") at our Henderson and Las Vegas locations. This Plan is NOT dental insurance. It is a membership program that provides access to included preventive services and discounts on additional dental treatments at SDC locations only.
The Plan is not transferable to other dental offices or providers. Benefits may not be combined with dental insurance or any other discount plan.
The following services are NOT included in any membership tier and will be billed separately at the applicable member discount rate:
The 10% member discount applies to all additional treatments performed at SDC not listed as included benefits. Discounts cannot be combined with insurance, CareCredit promotional rates, Cherry financing promotions, or any other offers.
Annual memberships are paid in full at the time of enrollment. Annual plans are non-refundable once any included service has been used. If no services have been used, a full refund may be requested within 7 days of enrollment. Annual plans auto-renew at the then-current annual rate unless cancelled in writing at least 30 days prior to the renewal date.
Monthly memberships require a minimum 12-month commitment. By enrolling in a monthly plan, you agree to 12 consecutive monthly payments. A valid credit card must be kept on file at all times. Payments are automatically charged on the same date each month.
Important: Monthly plan members who cancel before completing 12 months of payments are responsible for the difference between the retail value of services already received and the total amount paid to date.
For example: If a member has paid 3 months ($147) but has received services with a retail value of $431 (2 cleanings + 2 exams + x-rays), the member owes $284 upon cancellation.
SDC reserves the right to send any outstanding balance to collections if not paid within 30 days of cancellation.
Unused included services do not carry over from one membership year to the next. No refund or credit is issued for unused benefits. Included services must be used within the 12-month membership year in which they were issued.
To cancel your membership, written notice must be submitted to SDC via email at sdcoffices@gmail.com or in person at either SDC location. Cancellation requests are processed within 5 business days. Membership benefits remain active through the end of the current paid period. No partial-month refunds are issued for monthly plans.
Annual plan members who cancel after using any included service are not entitled to a refund of any portion of the annual fee.
SDC reserves the right to modify membership pricing, included services, or discount rates at any time. Members will be notified of any material changes at least 30 days in advance via the email address on file. Continued use of membership benefits after the notice period constitutes acceptance of the updated terms. Members who do not wish to accept the new terms may cancel without early cancellation penalties during the 30-day notice period.
By enrolling in the SDC Membership Plan and typing your full name as a digital signature on the enrollment form, you confirm that you have read, understood, and agree to all terms and conditions stated herein. Your typed name constitutes a legally binding electronic signature under Nevada law (NRS 719).
