• Monthly plans require a 12-month commitment. Early cancellation may result in a balance owed based on the retail value of services used minus payments made.
• After 2 failed payments, your membership is cancelled and any outstanding balance becomes due immediately.
• Unused benefits do not roll over. This plan is not insurance and cannot be combined with insurance benefits.
• SDC reserves the right to charge your card on file for any balance owed upon early cancellation or default.
